Benefits - What is Covered

The health care items or services covered under a health insurance plan.

Covered benefits and excluded services are defined in the health insurance plan's coverage documents.

Your plan should at least provide you with a Summary of Benefits and Coverage (SBC).

♦ All plans include things such as office visits, testing and surgical procedures.

Your portion of the costs for these services will be stated in the SBC.
