Usual, Customary, and Reasonable

Also referred to as Reasonable and Customary, this is the amount paid for a medical service in a geographic area based on what providers in the area usually charge for the same or similar medical service.

The UCR amount is used to determine the allowed amount for out-of-network services.

If you disagree with your insurance company’s determination of UCR you have the right to appeal.
